🐶 Puppy Adoption Application Questionnaire

1. Why are you interested in adopting a puppy at this time? (Please share your motivation, what drew you to this specific breed, and what you’re looking for in a companion.)

2. Have you owned a dog before? If yes, what breed(s), and for how long? What happened to your previous pets (still living, rehomed, passed away, etc.)?

3. Do you currently have any other pets? Type, breed, and age of each Are they spayed/neutered and vaccinated? How do you expect them to react to a new puppy?

4. Who will be primarily responsible for the puppy’s care? (Feeding, training, exercise, grooming, vet visits, etc.) Are all household members in agreement about getting a puppy?

5. What is your living situation? Own or rent? House or apartment? Do you have a securely fenced yard or access to outdoor space? If renting, do you have permission from your landlord to have a dog?

6. How many hours per day will the puppy be alone? What arrangements will you make for care during long work hours, travel, or emergencies?

7. What training methods do you plan to use? (Crate training, potty training, obedience classes, etc.) Are you willing to invest in professional training if needed?

8. Are you financially prepared for the ongoing costs of owning a dog? (Veterinary care, grooming, food, pet insurance, training, boarding, etc.)

9. Do you have a preferred gender, coat color, or temperament? If your preferences are not available, are you open to other options?

10. Are you prepared for a 10–15+ year commitment with this dog? (Dogs require lifelong care—through lifestyle changes, moves, or life events.)
